The rescue method for a confined space isn't something an employer figures out after a worker goes down. By the time someone is unconscious in a tank, vault, or vessel, it's too late to decide how they'll get out. OSHA requires the rescue method, equipment, and (if needed) rescue service to be evaluated and arranged before entry begins, not improvised at the moment.
Short answer: Non-entry rescue removes an entrant without anyone else entering the permit space. Entry rescue requires a trained rescue service to physically enter and reach the entrant. OSHA requires non-entry retrieval wherever it can safely contribute to rescue. When retrieval equipment would increase risk or wouldn't work in that specific space, the employer must instead arrange qualified entry rescue capability before the entry starts.
This is based on 29 CFR 1910.146(k) (general industry) and 29 CFR 1926.1211 (construction).
A note on state plans: OSHA has approved several states - California, Michigan, Washington, and others; to run their own occupational safety programs. These must be at least as protective as federal OSHA, but some word their confined-space rules differently or add requirements. What Is Non-Entry Rescue?
Non-entry rescue is retrieval performed without a rescuer entering the permit space. An entrant connected to a retrieval line and harness is pulled out from outside, typically by the attendant, using a mechanical device, winch, or fixed anchor point set up before entry.
It isn't defined by owning any one piece of equipment. OSHA's language is performance-based: it requires a system that will actually work for the specific space, connected and ready before entry, not a tripod sitting in the truck or a winch that hasn't been rigged to the entrant.
What Is Entry Rescue?
Entry rescue is when a rescue service physically enters the permit space to reach, stabilize, and remove the entrant. It's required whenever non-entry retrieval isn't a safe or workable option.
Entry rescue is not: a coworker climbing in on instinct, an attendant entering before being relieved, calling 911 without evaluating whether that responder can actually do the job, owning rescue gear with no one trained to use it, or assuming a fire department can handle a technical confined-space rescue simply because it responds to emergencies generally.
OSHA's construction standard formally defines entry and non-entry rescue: non-entry rescue occurs when a rescue service, usually the attendant, retrieves an entrant without going in; entry rescue occurs when the rescue service enters. Under 1926.1211, the employer must evaluate a prospective service's ability to respond in an appropriate timeframe, its proficiency with the tasks and equipment, and its ability to function in that type of space; not just its willingness to show up.

The choice isn't about convenience or what's on hand, it's whether the method can safely remove an incapacitated entrant from that specific space.

Does OSHA Require Non-Entry Rescue?
Both standards require it, with the same two exceptions.
General industry — 1910.146(k)(3): Retrieval systems or methods shall be used whenever an entrant enters a permit space, unless the equipment would increase overall risk or wouldn't contribute to rescue.
Construction — 1926.1211: Non-entry rescue is required unless one of the same exceptions applies. If it's not selected, the employer must designate an entry rescue service. If it is selected, the employer must use the retrieval system and confirm, before entry, that emergency assistance would be available if retrieval fails.
This is not "OSHA prefers non-entry rescue" - both standards require it unless a documented exception applies.
When Can Non-Entry Retrieval Be Omitted?

Two exceptions control the analysis - nothing else.
Retrieval would increase the overall risk: a line entangling with a supplied-air line, pulling the entrant across sharp projections, multiple lines tangling, or the pull itself worsening the entrant's position.
Retrieval would not contribute to rescue: internal turns a straight-line pull can't follow, fixed obstructions or machinery in the path, the entrant's work location moving them outside the retrieval path, or a configuration that prevents the line from transmitting force.
Construction's standard explicitly bars unsuitable equipment, lines reasonably likely to entangle, or systems that won't work given the space's configuration. Neither exception is a blanket exemption; the employer must evaluate the actual space and task, not assume a category of space is automatically excused.
When Is Entry Rescue Necessary?
When non-entry rescue is ruled out under either exception, or circumstances during entry make external retrieval unworkable: the entrant is behind an obstruction, needs medical stabilization before being moved, pulling externally risks further injury, the line becomes trapped or disconnects, or the retrieval system fails mid-entry.
Some of these are conditions OSHA addresses directly, some are space-assessment factors worked out ahead of time, and some are technical rescue judgment calls for trained personnel, not a checklist an untrained coworker can run in the moment.
What Happens If Non-Entry Rescue Fails?
This construction requirement is often glossed over. Under 1926.1211, when non-entry rescue is selected, the employer must confirm before entry - not after something goes wrong; that emergency assistance would be available if retrieval fails. That means answering: Who provides emergency entry rescue? How are they summoned? Are they available during the entry window? Can they reach the entrant in an appropriate timeframe? Have they evaluated this space? Do they have suitable PPE and equipment? Can they access the entry point?
The selected rescue service must also agree to notify the employer immediately if it becomes unavailable, so the gap isn't discovered mid-entry.
OSHA Retrieval-System Requirements

Harness: each entrant uses a chest or full-body harness.

Line attachment: at the center of the back near shoulder level, above the head, or another point presenting a small enough profile for removal.

Wristlets/anklets: allowed only when the employer demonstrates a harness is infeasible or creates greater hazard, and these are the safest alternative — not a default substitute.

External connection: the line's other end attaches outside the space so rescue can begin the moment it's needed.

Vertical spaces over five feet: a mechanical retrieval device must be available.

Individual retrieval lines: OSHA has interpreted 1910.146(k)(3)(i) to require each entrant to use their own line, a detail most guidance skips. This applies to entrants, not automatically to rescuers who enter.
Is a Tripod Always Required?
No. OSHA requires a system that works for the space, not a specific device. OSHA has even addressed whether a lifting device can serve as the retrieval system - reinforcing that the requirement is functional, not a fixed equipment list. What's appropriate depends on vertical vs. horizontal access, opening size, anchor availability, and internal turns. A tripod is common, not universal.
Who Can Perform Non-Entry Rescue?
Attendants typically carry it out, as assigned in the employer's procedure, and can't take on duties that interfere with monitoring entrants. OSHA has confirmed an attendant may participate in non-entry rescue and assist an entry service from outside without interfering with monitoring. Standing near the winch doesn't make someone qualified to operate it, the employer must ensure the assigned person understands the duty.
Can an Attendant Enter for Rescue?
Only under limited conditions: the employer's program allows attendant rescue, the attendant is trained and equipped for it, and another attendant has relieved them first. Construction reflects the same logic, and OSHA's interpretation on IDLH and non-IDLH entry covers this in more detail. Neither "the attendant can never enter" nor "the attendant can enter whenever there's an emergency" is accurate.
Entry Rescue Service Requirements
Before designating any service, the employer must evaluate whether it can respond in an appropriate timeframe, is proficient with the tasks and equipment, can function in that type of space, is equipped and available during entry, and agrees to notify the employer if it becomes unavailable. OSHA's Appendix F to 1910.146 lays out non-mandatory evaluation criteria. The employer must also brief the service on hazards and give it access to the space for planning and practice.
There's no universal response-time number - no fixed "four minutes," no formula of adding minutes to a fire department's stated response. What's timely depends on the specific hazards.
Entry Rescue Training and Practice
OSHA requires rescue-specific PPE and training, training in assigned duties, authorized-entrant-level training, basic first aid/CPR training, at least one team member with a current first aid/CPR certification available, and practice rescue at least every 12 months using a person, dummy, or mannequin from the actual space or a representative one.
This does not mean every team member needs current CPR certification, that OSHA specifies a universal training-hour count, or that one annual drill proves proficiency for every space. Classroom instruction alone doesn't substitute for hands-on practice.
Is SCBA Required for Every Entry Rescue?
No. Respiratory protection must match the actual atmospheric hazard. For IDLH atmospheres, OSHA's respiratory protection standard, 1910.134, requires atmosphere-supplying respirators and immediate rescue capability, but equipment and staffing depend on the hazard assessment, not a blanket SCBA rule.

After a Rescue, Failed Retrieval, or Near Miss
A drill that goes wrong, a line that entangles, a rescue service that turns out unavailable, these signal a real program weakness whether or not anyone was hurt. Worth reviewing: a failed practice retrieval, equipment malfunction, an unauthorized rescue attempt, a service becoming unavailable, or an actual rescue or injury. Document what happened, preserve permit and equipment records, examine immediate and underlying causes, and correct or retrain before the next entry.
It's also worth understanding the difference between an incident and an accident ; a near miss that never becomes an injury can reveal the same underlying problem and deserves the same investigation.
